I totally agree it would be a worry though, in the US they would always advise that a woman with HIV should bottlefeed to be on the safe side. In Africa studies have found that the transmission rate is pretty low anyway, but if women follow certain protocols (on top of looking after their nipples not feeding the baby any solids until they are over like 8 months because earlier can cause tiny tears and abrasiions in their gut that can make transmission more likely if the mother were likely to have a bleed) that is is safer for an HIV infected mother to breastfeed. Bottlefeeding leads to much more death there. (Bottlefeeding is notoriously unsafe in areas with bad wter supplies and also it is too expensive in most cases for women to continue.)
I know we are not talking about Africa, but I just find this all interesting! Sorry for going off topic a little.
More on point - from what I have read, most women who make arrangements with someone to buy breastmilk (it is getting more and more common) usually have the woman have blood tests to be sure she is disease-free.
